Which of the following lipid profile results would most strongly merit referral to a specialist lipid clinic?
Non-HDL cholesterol of 8 mmol/L
LDL cholesterol of 5 mmol/L
Total cholesterol of 8.5 mmol/L
Triglyceride level of 5 mmol/L with an LDL cholesterol of 4 mmol/L
Triglyceride level of 7.5 mmol/L
Key Learning Point
Referral to lipid clinic for specialist assessment is indicated for: total cholesterol greater than 9 mmol/L, LDL cholesterol of greater than 6.5 mmol/L, non-HDL cholesterol of greater than 7.5 mmol/L and fasting triglycerides of greater than 10mmol/L.
Specialist lipid clinics play an important role in the management of more 'adverse' lipid profiles; some of which may have a familial aetiology. More marked elevations in lipid profile components can translate into a much greater risk of cardiovascular disease and may require more aggressive treatment to reduce this risk. Hypertriglyceridaemia is also a risk factor for pancreatitis.
The figures outlined in the learning point highlight what levels of total cholesterol, LDL cholesterol, non-HDL cholesterol would merit referral to lipid clinic.
