Demo Question

A 24-year-old low-risk female in her first pregnancy presents to her GP for review. According to previous scans she is 25 weeks pregnant and the pregnancy has been progressing fine.

On examination, symphysis fundal height (SFH) is 31 cm, fetal heart sounds are heard and her blood pressure is 102/74 mmHg with normal urinalysis.

Which of the following is the most appropriate action for this patient?

Refer for immediate review in hospital antenatal clinic

Continue to monitor progress in antenatal clinic

Refer for immediate review of community midwife

  Refer to radiology for ultrasound

Refer for routine appointment at the hospital antenatal clinic

Key learning points

Causes of increased SFH include polyhydramnios, congenital malformations and multips.

Explanation

This woman has an SFH which is 6 cm more than her dates.

This should raise alarm bells as an SFH 2 cm above or below dates after 20 weeks is abnormal and merits further investigation.

Causes of increased SFH include wrong dates, full bladder, polyhydramnios, congenital malformations and multiple pregnancy.
